Crisis Intervention Program
Guidelines for Assistance:
CPN will review all requests for in-kind assistance from individuals or families living with PKU. CPN offers aid in the form of products, such as low-protein foods, formula, digital scales and other dietary aids used in the treatment of PKU. Aid is not provided in the form of money and is intended as short-term interim assistance, until permanent funding can be obtained. The amount of funds for this program are limited to the amount of budgeted donations.
1. Requests for in-kind assistance must be made in writing.
2. Anyone may apply. However, a letter from a health professional, indicating diagnosis is required. This is to verify the applicant’s PKU status or other metabolic disorder.
3. Request letter should include the following information:
- The individual’s name and address.
- Parent’s name and address, if individual is a minor.
- Previous year’s federal tax return; if you do not file a tax return, request a financial status form from us.
- Annual expense of treatment.
- Any other additional financial hardships or extenuating circumstances that should be considered.4. Assistance will be based on the needs of the individual/family and the availability of funds.
5. Applications are reviewed once per month, funds permitting.
6. Applicants who have not received funds from us previously will be given the highest priority. However, the Board of Directors reserves the right to award more than once to the same family. The amount of assistance will vary, depending on availability of funds.

Scholarship Applications
In 1995 Children's PKU Network instituted a college scholarship program which awards two scholarships per year ( for $1,250 each) funds permitting, to qualified young adults with PKU. For the calendar year 2000 Ross Products Division of Abbott Laboratories and the maker of Phenex 1& 2 for infants, children and adults with PKU has funded this program. CPN currently has over 100 articles and research studies on file related to PKU and other rare disorders.
